04/2022-Present Affinity Hospice Care: Triage RN

Triage RN (Remote Station): Responsibilities include, but not limited to supporting clients / nursing field staff after hours and collaborating with colleagues, physicians, and vendors to provide for patient needs.

Triage RN tasks: Answers telephone calls promptly. When able should accept transfer of calls from the answering service. Keep up with communications regarding specific agencies, patients, on-call staff, schedules via email, text, phone calls. Maintain HIPPA compliance per agency guidance. Maintain positive and supportive demeaner in all interactions, including employees, physicians, facility staff, vendors, patients, and families. Provide appropriate advice for symptom management. Use nursing knowledge, experience, and research to support practice. Dispatch on-call nurses to visit patients as appropriate. Navigate technology tools used in the triage position. Document appropriately in EMR, emails, and end of shift reporting. Process patients’ Admission visit documentation / ladders and assist with entering / maintaining patient calendar schedules. Attend team remote meetings and trainings as requested.

04/2021-04/2022 Affinity Hospice Care: Case Manager / On-Call Nurse

RN Case Manager / On-Call Nurse: Responsibilities include, but not limited to assisting the Team with Case Management/Visit. Registered Nurse RN Job duties can include but are not limited to: Completing an initial assessment of patient and family to determine hospice care needs. Regularly re-evaluates the patients’ nursing needs. Use assessment data to determine care plan needs. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated. Registered Nurse Counsels the patient and family in meeting end of life needs. Initiates appropriate preventative nursing procedures. Collaborate closely with HOSPTIALISTs and RN staff to provide Hospice Case Management services to hospital-based hospices GIP patients. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition. Supervises the Hospice Aides and LPN's.
