I am a Licensed Chiropractor and would like to be your new associate. I am looking for a position within a practice that is looking for a team player. I was the lead doctor in a multidisciplinary PI practice for over four years and continued to practice mostly PI cases. Now I'm looking for growth and opportunities. I am AHCA certified.

Administrative Follow HIPPA patient privacy guidelines. Conduct and follow through on patient case management, attend staff meetings, supervision of subordinate personnel, communicate patient care plans with the coordinator for scheduling purposes, understand patient insurance benefits and financial responsibilities. Refer patients to coordinator with their questions as to finances and communicate with Coordinator to ensure patient financials are in good standing. Communicate with attorneys regarding Personal Injury patient care and treatment plan.

Provide reports as needed and required. Clinical Perform new patient evaluations and consultations, perform ROF, and review MRI’s. Use proper DX codes, change DX codes as needed, perform re-evaluation as required. Perform manipulations and modalities including but limited to: Rock Tape, PT, e-stim, heat/ice, cold laser, IST, ultrasound, posture pump, cervical traction, Spinal decompression, explain & demonstrate at home exercises or therapies.

Preparation of medical documents on time, co-manage and communicate treatment plans with all people involved including physical therapists, massage therapists, etc., to ensure the best possible outcome for the patient. Refer patients out for treatment as applicable. Maintain patient files to include but not limited to: complete patient information, accident information, health history, wellness surveys. Consent & Authorizations, signatures, financial forms, payments, clinical notes, SOAP notes, lab reports, & referral forms. Communication Educate patients on the benefits of chiropractic care, and all services provided by the office. Explain patient care plans so that the patient understands what is expected from them and their treatment goals. Prior to treatment, explain manipulations and modalities as well as comprehensive care plan.

Personal Injury

These cases require additional documentation. Payment services: Med Pay, major medical, as well as: File Management Date of accident (DOA), contact information, attorney name and all communication patient & professional including patient compliance, new patient packet, patient information sheet, health history, case & social history, patient job description, new problems, re-evaluations, physical/regional exam forms, SOAP notes, diagnostic forms, informed consent, practice forms, managed care, surveys & questionnaires forms, exercise reports, work/school letters, insurance assignment, information release and payment agreement, lien, authorization to release medical records, workers compensation authorization, X-ray consent, reports x-ray & MRI reports, request slips, PI financial agreement, contact information, referrals to other doctors, MRI'S, and justification for such.

Examinations include but are not limited to: Chief complaint, review of history, addressing physical complaints, onset, provocation/palliation, quality of pain, region and radiation, severity/site, timing (OPQRST), orthopedic, neurological, asymmetry/misalignment, ROM, tonicity changes. Patient forms: patient information sheet, health history check list, case history record, social history sheet, patient job description, new problem/re-evaluation form, SOAP notes, patient survey, difficulty in performing Activities of Daily Living (ADL), difficulty in home care, self-help activities form, exercise instructions, home care & exercise report, disability certificate, work/PE excuse letter, work/school/doctor’s appointment letter, failure to follow advice letter. Physical Exam Forms: physical exam legend/key, regional exam forms, physical examination letter, PI questionnaire. Insurance forms: release of claims, insurance assignment, information release & payment agreement, workers compensation authorization form, workers compensation history form. Diagnostic forms: pregnancy warning & consent to X-ray form, imaging request slip, MRI history sheet, X-ray reports, lab requests & imaging interpretation reports, all lab forms & requests.

Referring patients to other chiropractors and professionals, if needed, while using the application of Light Therapy, Color Therapy, and Music Therapy for those patients.
