Support families who have been successfully discharged from DHS/CUA formal case-management services by providing reintegration supports, consultations and community resources

that will decrease the probability that the family will re-enter the child welfare system. While working closely with all members of the assigned case management team prior to successful discharge; with children, biological families, and with external agencies and partners.
