Provides comprehensive admissions health assessment, medication reconciliation, and patient education, makes interdisciplinary care recommendations, provides case management and follow-up skilled nursing for patients and ensures compliance with state Registered Nurse Practice Act, CMS standards of care, and governmental laws and regulations.
Confers with physicians in developing treatment plans based on physician orders and initial patient assessments.
