03/2015 - 05/2015 Substance Abuse Counselor, Girard Medical Center Philadelphia, PA I was hired through an agency to temporarily handle a caseload of patients in a long-term, inpatient medically managed (Methadone) treatment setting. The paperwork I was responsible for included: writing treatment plans, clinical summaries, discharge summaries, group and individual notes and aftercare plans. I signed-off on requests for dosage increases. I ran group sessions two times per week, and facilitated a topic group weekly. I saw patients for individual sessions weekly, as well. I wrote up initial and concurrent reviews which I provided to our Utilization Review specialist so she could work with representatives from CBH and BHSI to obtain approval for continued coverage for their treatment. I maintained contact with parties whom my patients had signed a release of information for me to speak with, such as: family members and P.O.'s. I invited the family members to participate in treatment. I also contacted agencies such as O.A.S. and attempted to secure beds in recovery houses for any patients who were completing treatment and had nowhere else to go in their early recovery. 05/2007 - 08/2014 Substance Abuse Counselor, Livengrin Foundation Bensalem, PA My job consisted of handling a caseload of patients who were being treated for substance abuse in an outpatient setting. My normal daily responsibilities included: writing treatment plans, clinical summaries, discharge summaries, group and individual notes and aftercare plans. I ran an intensive outpatient group three times per week, and a general outpatient group once per week. I saw patients for individual sessions, which sometimes included family members. I performed intake assessments of new patients, during which I evaluated them for the proper level of care. I worked with the representatives from our patients' insurance companies to obtain approval of coverage for their treatment. Another function I served was as a liaison for twelve-step groups that entered our facility to conduct meetings.
