Responsibilities included administering comprehensive speech and language evaluations for children from birth through 21 years, in addition to evaluations of adults, and reporting all information retrieved from interview, observation as well as informal and formal testing. In addition, it was the responsibility of the Speech Pathologist to maintain copies of the evaluations for the Bureaus for a minimum of one year and to provide additional information of the Clinical Evaluation team as may be requested. September, 1988 to 2012: West Harvey-Dixmoor School District 147. Responsibilities included providing speech and language therapy and diagnostic services for children from Pre-k through 8th grade with a variety of disorders including phonological impairment, stuttering, language delay, language impairment, developmental delay, cleft palate, palatal insufficiency, hearing loss, auditory processing disorders, specific learning disorders, and emotional disorders. In addition, responsibilities included evaluation and timely reevaluation of children referred for case study or who have received services. All services provided by the Speech Pathologist were documented for Medicaid Billing. The Speech pathologist was also responsible for maintaining Eligibility and IEP records for each child receiving speech/language services.
