DIRECT DEPOSIT/W-2/FEE POLICIES

1. ANY CHANGE OF ADDRESS OR BANKING INFORMATION MUST BE MADE IN WRITING. YOU MAY SEND IT BY E-MAIL OR BY HAND. 2. If you have a bank account or pre-existing paycard you would like to use for Direct Deposit, please provide the information either by email or by hand. This can be given or sent any time between your start date and the Monday of your second week on the job. If we do not receive direct deposit information by 5pm Monday, you will be issued a paycard, which can be picked up at the Staff Finders office in Troy. 3. If a direct deposit needs to be cancelled during a payroll week after it has been sent out, or a check is lost, damaged, or stolen and is unable to be cashed, employee will be responsible to pay $40.00 for a stop payment. A new direct deposit or check can only be issued 48 hours after the stop payment has been made, and it will have the $40.00 deducted from it. 4. For every direct deposit, there will be an email weekly with a link to access the paystub digitally. It is your responsibility to keep track of your paystubs – Please DO NOT delete the paystub emails. If you do not receive it by Friday, call the office as soon as possible for it to be resent. Any request for additional copies of paystubs after the Monday following payroll will incur a $2 administrative fee per paystub. 5. If any garnishments are ordered to be taken out of your direct deposit, there will be a one-time administrative charge per month. For electronic payment, i.e. direct deposit, there will be a $2/month charge. 6. If you request to change your State/Federal tax paperwork outside of a QLE, or Qualifying Life Event (marriage, birth of a child, etc.), you must fill out a new tax form. 10. If you are on direct deposit and your assignment ends (voluntary or involuntary), your direct deposit will no longer be valid after 4 consecutive weeks following your last payroll deposit. You MUST notify SFI in writing (or via email) to reactivate your direct deposit, or you will be issued a pay card.

11. If you change your bank/bank account number, you MUST notify SFI immediately. The “drop dead” time to notify SFI is Monday by the end of business day for the payroll week in question it should be in effect. 12. MICHIGAN PAID MEDICAL LEAVE ACT (PMLA)

Effective March 29, 2019, Michigan's Paid Medical Leave Act (PMLA) requires covered employers to provide paid medical leave to eligible employees.

Employees accrue one hour of paid medical leave for every 35 hours worked. Employees use of paid medical leave cannot exceed 40 hours per year. Under the PMLA, Staff Finders is allowing employees to accrue paid medical leave of no more than one hour per calendar week (as long as employee works minimum of 35 hours in week). Employees begin to accrue paid medical leave on March 29, 2019, or upon hire, whichever date is later. Employees do not accrue paid medical leave for hours taken off work for paid leave (including paid vacation days, paid personal days and paid time off). Employees are allowed to carry over unused, accrued paid medical time from year to year, up to a limit of 40 hours per year. Staff Finders employees will be required to wait 90 calendar days after beginning employment to start using accrued paid medical leave. Employees May Use Accrued Medical Leave Due to Any of the Following Reasons: 1. An employee’s own mental or physical illness, injury or health condition (including the need for medical diagnosis), care or treatment and preventive medical care

2. A family member’s mental or physical illness, injury or health condition (including the family member’s need for medical diagnosis(, care or treatment and preventive care

3. Closure of the employee’s primary workplace by order of a public official due to a public health emergency or closure of an employee’s child’s school or place of care by order of a public official due to a public health emergency, or when it has been determined by health authorities that the employee’s (or the employee’s family member’s) presence in the community would jeopardize the health of the others due to a communicable disease

4. If the employee or an employee’s family member is a victim of domestic violence or sexual assault, to obtain medical care, psychological or other counseling for physical or psychological injury or disability, or to:

EMPLOYEE REQUIREMENTS

Staff Finders Employee Requirements to qualify for Paid Time Off (PTO) under the Paid Medical Leave Act (PMLA).

● Employee must provide Staff Finders written (E-Mail) notice prior to normal shift start time on day they wish to use PTO. Request for time off can be emailed to ad2iqd@r.postjobfree.com or to your main recruiter’s email address.

● Employee who requests PTO may use the accrued hours in no less than half- or full-day increments.

● Employee must provide authorized documentation to Staff Finders within three calendar days of requested day off.

● Failure to comply with listed above policies may disqualify employee from PTO under the PMLA. Staff Finders is required by law to maintain the confidentiality of health, domestic violence and sexual assault information about an employee or his or her family member and cannot disclose the information to others without the employee’s permission. Staff Finders is not required to include overtime pay, holiday pay, bonuses, commissions, supplemental pay, piece-rate pay or gratuities in the calculation of an employee’s normal hourly wage.

If employee stops working or is terminated at an assignment assigned by Staff Finders and is re-hired to another assignment with Staff Finders at any time, any accrued hours under the PMLA are not retained. Employee will start back with zero accrued PTO hours when new assignment begins

RULES OF CONDUCT

People work together best in an atmosphere where they are valued as individuals and recognized as key members of a team. It is important that as a member of that team you understand what is expected of you. If you have any questions at all, discuss it with your immediate supervisor or SFI. Understanding is vital to teamwork. The following are examples of employee conduct that is not permitted and will result in termination of employment and could affect your unemployment claim.

● Poor attitude and/or insubordination

● Failure to maintain satisfactory work performance standards

● Falsification of time records, personnel records or other company records

● Smoking, gambling, use of drugs or alcohol on company premises

● Sleeping on the job

● Conducting ANY illegal activity while on SFI or SFI clients’ premises (regardless of whether you are “on the clock” or not)

● Possession of firearms, explosives or other lethal weapons on the premises including the parking lot

● Abuse or waste of company tools, equipment, fixtures, property or supplies

● Creating or contributing to hazardous, unhealthy or unsanitary conditions

● Failure to cooperate with supervisor or co workers

● Disorderly conduct, harassment of other employees including sexual harassment or use of abusive language

● Fighting, encouraging a fight, or threating, attempting or causing injury to another person on or off the premises when engage in a work assignment

● Dishonesty or the unauthorized possession, removal or use of company or another employee’s property, including records and confidential information

● Repeated tardiness or absence without prior notification to Supervisor or without satisfactory reason

● Quitting an assignment or walking off during shift without approval, regardless of intent to quit

● Unavailability for work, refusing or ignoring an offer of work
