As a counselor my job was to provide direct services and support for clients. This included treatment planning, detox support, case management which included progress development and crisis intervention. I assisted clients with transportation information to facilitate attendance in the methadone program. The clients were also given referrals throughout their care as needed and at discharge. Individual and group counseling were also an essential component in the treatment of the client. Client education and crisis intervention played an important part in supporting the client in the recovery process. As a whole contributing to and collaborating with colleagues during weekly team meetings, our goal was to ensure the most effective treatment planning for the client.

As an additional source of support, I provide medical, psychological & substance use disorder resources for clients. Included in the discharge planning for the clients I also provide housing support through HRA. I provide information and connections to relapse prevention support groups e.i. 12 Step programs, NA, AA, psychotherapy support groups, church or other religious and supportive organizations.

Arranged coordination of care to optimize patients’ health goals. This included selecting homecare agencies to provide aides in the home to help with basic ADL’s, arranging transportation for physical therapy and doctor appointments, locating and arranging adult daycare visits to family members, day trips, churches for social activity. Coordinated with hospice for palliative care services including, religious, nursing and comfort care while respecting the dignity of the patient.
