Nurse Manager - Job Duties/Responsibilities:

Ensures Baylor College of Medicine's (BCM) policies and procedures are followed.

Promotes and encourages an environment that provides quality patient care, patient and staff safety.

Supervises personnel objectives by selecting, orienting, training, assigning duties, scheduling, coaching, and counseling and may discipline employees.

Defines work responsibilities and prioritization, setting performance development plans and goals, and conducts annual performance evaluations and may make recommendations for staff clinical advancement.

Implements accountability measures and timelines to meet clinical goals.

Accounts for routine decisions including shift oversight to include staff and resource utilization related to the delivery of high-quality care and customer service.

Oversees 107 Baylor Neonatology Faculty within the Division currently registered in TxEVER.

Partners with BCM Neonatology Faculty to identify appropriate physician of record to complete the Medical Data Entry in TxEVER. Contacts funeral home to assist with reassignment, if necessary.

Neonatal Intensive Care Unit Nurse Care Coordinator – Job Duties/Responsibilities

Performs a comprehensive goal-focused and individualized assessment for patients referred for evaluation, to include patient/family’s biophysical, psychosocial, environmental, discharge planning needs and/or financial status.

Performs and documents Discharge Planning Assessment within 72 hours of patient’s admission to the NICU.

Performs chart reviews, identifies care coordination needs and barriers to discharge. Documents Care Coordination Progress Notes.

Proactively participates in the planning and coordinating services across the continuum of care.

Coordinates with interdisciplinary care team for the provision of continued care needs as required. Serves as advocate on behalf of patients and families on an ongoing basis across the continuum of care (addressing in/outpatient needs).

Utilizes face-to-face meetings, routine family interactions and interviews to confirm understanding of the comprehensive treatment plan, validating educational understanding.

Composes letters or completes forms needed by the patient’s family. Arranges for translation to another language with Language Services.

Schedules subspecialty and developmental follow up appointments once outpatient referral orders are place in Epic. Ensures Early Childhood Intervention (ECI) referrals are completed.

Receives and acknowledges Epic orders for outpatient Home Care services.

Provides a Vendor Choice Letter for the patient’s family for selection of a home care company for Durable Medical Equipment (DME) and/or physical, speech and occupational therapies.

Completes Final Assessment at discharge.

Completes Care Coordination Hand Off Note and sends to PCP.

Communicates with families after discharge via telephone to ensure safe transition from the hospital to home, answer any questions and assist with any concerns. Documents Post Discharge Follow Up by telephone (24-48 hours post discharge). Serves as a communication link between inpatient and outpatient services as well as across the various health professionals involved in an individual patient’s care as needs arise.
