Discharge Coordinator

Short term rehab. Discharge process per policy and procedure and Medicare Rules and Regulations. Responsible for the transfer/discharge process whereby residents are transitioned smoothly to higher levels of care, as required. Assist patients and families with placement to facility of choice. Make sure transition to home is appropriate, home visits if required to establish safety and cleanliness. Direct/ lead patients and families in Care Plan meetings, document everything. Social Worker duties performed when required: assist with Medicare/Medicaid apps, and other state assistance.
