As an Intake Counselor of an Opiate Dependence, Methadone clinic, I am the first to see the patient. It is important that you observe, listen and collect pertinent information for assessment for patient’s needs and abilities. My position demands that I collaborate and obtain certain information’s in reference to the patient: (such as their medical history needs, their emotional stability, financial (poverty or wealth status) as well as their psychological and social status) As an Intake Counselor, I am responsible for the collection of data and to prepare a confidential chart for the patient and assign a case number to that patient. I am to assist the patient in filling out required documents for their records and to assign patient to a Primary Counselor. I am also responsible for the lab collection, for drug testing. This is either by way of a urinary screen or cheek swabbing. The screening is assured that patients are not actively using illicit drugs while in treatment. Some testing might include pregnancy testing, the levels of opiates or other licit or illicit drugs. I am also responsible for the DSM IV criterion for substance use disorders both dependency and abuse, ASAM Six Dimensional Criteria scale/ Level of Care, Initial Treatment Plan, Biopsychosocial Assessment, patient Safety education and Take home Justification, in which is mandated by the State and Federal Agencies.
