●Function as case navigator. Provide clear and detailed instructions to assist the customers and funeral directors regarding their requests. This includes providing written guidance to customers and hospitals in clear and courteous plain language and responding to inquiries.

●Processing errors are reported a supervisor promptly with accuracy.

Follow directives promptly and asks questions when instructions are unclear.

All certificate(s) issued are reconciled and matched with the correct order.

Manual certificates must be carefully reviewed for completeness and accuracy prior to registration.

●Manual registrations are carefully entered with accuracy to prevent data entry errors, resulting in a correction.

●Review faxed documents and requests for processing.

●All registration queues are reviewed constantly to identify cases pending registration. ●Assist in the retrieval, reproduction and archival of death/birth certificates.

●Discrepancies identified during and/or after the reconciliation must be reported immediately to a supervisor in accordance with department guidelines.

●Records and reconciles security paper document numbers in the electronic system and in the logbook.

●Complete security paper reconciliation at the end of each shift.

●Void security paper according to department guidelines.

●Receives birth information, including Acknowledgement of Parentage (AOP) from NYC area hospitals and home births, verifying completeness, obtaining missing information, and registering/filing as appropriate.

●Certificates and permits should accurately indicate the file date and time and the certificate/log number after registration. Replacement burial permits with the required supporting documentation must be validated and issued in accordance with DOH guidelines.

●Registration pending Queue. Complex issues and problems are escalated promptly to the supervisor or director to prevent registration delays.

●Batch registered Manual certificates for submission to the Records Management Unit.

●Documents must be carefully and accurately identified, archived, or reproduced in a timely manner in accordance with DOH guidelines. All birth and death, transit, and review and collect all applications.

●Performs trainings, translate and other routine tasks as required by the Supervisor and Unit Director.
