Kaiser Emergency Care Management HUB

-Responsibilities included collaboration with Internal Medicine Hospitalists for admitted Kaiser patients at non-Kaiser hospitals. This position was remote and daily I communicated with other RNs at various hospital facilities and obtained the names and numbers for attending physicians at non-Kaiser hospitals so that our doctors were able perform a Peer-to-Peer review with non-Kaiser physicians. This information was documented in each patients’ chart for our physicians to review. Our primary goal was to repatriate patients, once stabilized from non-Kaiser hospitals to Kaiser hospitals. Once it was determined that a patient could be repatriated, I contacted the Emory Transfer Center in order to obtain a bed assignment at Kaiser hospitals. Also arranged transportation for patients that were able to be repatriated and communicated with other emergency care management staff daily. It was my responsibility to create a listing for all on-call specialty physicians such as Infectious Disease, Oncology and the on-call admission physicians for Emory Midtown and Emory St. Joseph’s hospitals. -Urgent Care Nurse for employees and staff members at Coral Industries. Practiced as a “PRN” employee health nurse and my responsibilities included addressing acute and or chronic conditions for all staff members, mainly those that performed glass installation. For employees with chronic health conditions such as hypertension and diabetes, I would also ensure that they were following their plan care provided by their primary care physicians. This was performed by making sure staff members understood their plan of care by answering questions regarding chronic health conditions. This included monitoring of blood pressures, blood glucose levels and teaching was provided to ensure that employees understood how to operate glucometers and by encouraging those with hypertension to invest in blood pressure cuffs to monitor their blood pressure at home. Therefore, my job consisted of a great deal of health education. All acute injuries sustained while working were officially reported to the office manager, Mrs. Blackburn so that the employee was referred to the nearest hospital and or acute care facility to receive treatment for on-the-job injuries. As the acute employee health nurse my responsibilities consisted of employee triage in the event of an injury, ensuring all working staff members that performed glass installation were advised on safely installing glass and or glass shower doors safely in order to prevent injury. However, in the event of an injury, as the employee health RN it was my responsibility to assess all acute injuries with documentation of injuries and providing follow up care as needed. Most injuries included sprains and or fractures, superficial and deep lacerations. Injuries sustained by staff members that I was unable to address and or were out of my scope of practice were advised to seek treatment at a facility of a higher level of care such as an area hospital.

-Worked remotely as an outpatient and inpatient RN Case Manager for United Healthcare. Responsibilties included reaching out to members with United Healthcare Insurance daily by phone to inform them of various benefits available with their insurance plan. Ensuring that patients had the proper follow up appointments with primary care providers and specialists. Providing patient education daily based upon past and current chronic or acute conditions in order to prevent unnecessary hospital admissions. Recognizing and addressing “GAPS'' in care while assisting with management of chronic versus acute illnesses. -Responsibilities include collaboration with Internal Medicine Hospitalist and other specialties such as Infectious Disease, Surgical Oncology and Nephrology to ensure that the patient is without any gaps of care during the hospital admission. Implementation of appropriate, safe discharge care plans which may include home with home IV antibiotic therapy, inpatient/outpatient short-term rehabilitation, home-health services, arranging inpatient/outpatient Hospice, providing durable medical equipment as indicated and ensuring that the patient’s family is well informed during the admission process and especially the discharge care plan. Creating attainable discharge plans for patient(s) to reduce the risk for readmission and encouraging outpatient compliance with primary care and other specialties. Collaborating directly with contracted vendors to assist in attaining a safe, realistic discharge plan. -Responsibilities: Lead Healthcare Team RN for Adult Internal Medicine, also cross-trained for Pediatrics for primary care. Focused on preventative care and management of chronic illnesses such as hypertension, diabetes, hyperlipidemia while assisting in the prevention disease progression. Served as a crucial resource for the healthcare team which includes a LPN, physician, and Kaiser members. Participated in conscious sedation with each assignment physician. Responsibilities all included inventory of medication for the department and maintaining records for managerial review or audit. Provides teaching for members that are newly diagnosed with Diabetes, which includes diet, medication management, self-insulin administration if indicated and providing the member with resources such as classes and other forms of outreach. Awareness of HEDIS measurements to assist in preventative care along with testing such as mammograms, PAP SMEARS, Diabetic Retinal Exams, and Immunizations. The City of Refuge Clinic allowed patients to obtain their blood pressure and diabetes medications for free or at a more affordable rate. Assisted homeless families in enrolling their children in schools while ensuring that they had all required vaccine documentation. Mercy Care also provided free HIV medication and counseling for those in need in the Atlanta area. Offered free STD/HIV testing, pap smears and mammograms to promote early detection and treatment of breast or cervical cancer. -Emory Adventist Hospital ER was a small facility with approximately 12 to 14 beds and although most of our patient population didn’t exhibit severe acuity, however on occasion there were patients in our department with severe acuity who required transportation to larger facilities in order to obtain a higher level of care. Responsibilities included RN patient triage, charge nursing and initiation of standing order protocols while working in conjunction with the ER attending to provide immediate care for patients. Team-work oriented environment and practiced appropriate RN delegation when needed. Constant communication with other departments such as lab, x-ray, and CT, especially during after-hours because most techs for these departments had to be called in to perform most radiology imaging. Therefore, as the RN, it was my responsibility to ensure that the departments were alerted for diagnostics to be performed in a timely manner. Initiation of IV(s), IV therapy, administering of PO/IM/IV and assisting physicians with the insertion of central lines. Also performed EKGs, assisted with pelvic exams, conscious sedations, cardiovascular monitoring for patients being held for ICU admission or patients awaiting to be transferred to a facility of higher care. -Worked as a travel RN for emergency rooms in various states including Minnesota, D.C., and New York. Also completed two work action assignments for U.S. Nursing in Minnesota and the District of Columbia. Orientation is minimal when working as a travel RN during work action assignments and therefore the expectations for independence with efficiency while maintaining all standard levels of care. In addition to the work action assignments, I completed an 8-week contract at Washington Hospital Center ER in Washington, D.C., a level One trauma facility and Burn Unit. The ER had sixty beds and the acuity levels were extremely complex. The facility had an extremely high population of HIV positive patients due to the city’s exponential rate of IV drug use. Care populations also included acutely/chronically ill patients with hypertensive emergency, CVAs, acute MI, Cancer and Diabetic Keto-Acidosis also known as DKA. Responsibilities included initiation of standing nurse protocols, difficult IV insertion, administering vasopressors, assisting physicians with conscious sedation, performing EKGs, foley catheter insertion/care and stabilization of ICU holds until transferred to the unit. Ensuring that patients had ordered diagnostics such as x-rays, CT and MRIs were performed in a timely manner and communication of results with the ER physicians.

-Work-action assignments (RN Strikes) completed included a 24-hour nursing strike in Minneapolis, MN and a 5-day strike in Washington, D.C. Assigned to work in the ER during both strikes. Although orientation was extremely minimal, safety was always my priority and I was able to function efficiently, work independently and most importantly provide safe treatment for all patients. Responsibilities remained in consistency as an ER nurse, caring for sub-acute and or acute patients by providing immediate intervention and care while working in unfamiliar environments. -Completed a 13-week assignment in the ER at the Coliseum Medical Center in Macon, Ga. Functioned independently in a level 2 trauma center and twenty-four bed emergency department. Although there as a traveler, I functioned well with the permanent hospital staff through open communication and embracing teamwork. Delegated task to ancillary staff when appropriate, worked closely with ER physicians and mid-level providers. Often assigned to the urgent care area of the ER which included eight beds and was supervised by the assigned Charge RN, who also closely observed triage and quickly moved urgent care patients from the waiting area to an exam room for evaluation. -Began my ER residency at EUHM in the emergency department and for a course of 3 months I shadowed an RN as well as collaborating closely with other ancillary staff to become accustomed to my new profession. After residency, I remained at EUHM for 8 years as a staff RN, charge nurse and preceptor. During this time, the hospital was migrating from paper charting to electronic medical records, so the ability to adapt was extremely important. Responsibilities were consistent with working as an ER nurse and included working closely with ER attending physicians and residents, utilizing standing protocols, nurse triage and functioned as assigned charge nurse, ensuring that the department is functioning safely and effectively. Leadership skills were valuable as charge nurse because responsibilities increased such as directing EMS traffic, learning to be creative in providing safe patient care even when staffing is low, and the volume is high. Remaining aware of patients that were considered level one and ICU bound while ICU beds were unavailable. When this situation would occur, my focus was to remain as a constant resource for the nurse caring for the critical patient and offering support for the other patients that they are responsible for at that time. -Worked in the Grady ECC as a nurse extern between my junior and senior year of college and continued until the beginning of my residency at EUHM. Attended EKG, phlebotomy and other ER educational classes that were extremely helpful and allowed me to connect what I had learned in class and applying it into practice for patient care.
