I am working on a Medical/Surgical floor. I have experience with pulmonary, cardiac, neuro, Gi, Gu and post operative patients that are stable. The patient load is 5 to 6 patients per shift. I have also been a charge nurse on this floor as well.

Absolute Care (November 8, 2021- February 1,2023)

Case Manager – Physical Care

Responsible for a panel of 80 to 100 patients or members of Absolute Care which provides concierge medicine for the under privileged. The goal is to meet the patients where they are to diminish the social determinants of health that are preventing the patient from being successful with maintaining their health.

This is done by assisting the patient with filling out disability paperwork, TDAP forms, housing applications.

I also coordinate transition from the hospital back to the outpatient environment.

In addition; I schedule in clinic appointments to review medications, follow up on care questions.

Develop and continue their care plans and make adjustments as needed.

I have patients that are difficult to reach and work with our outreach community health workers to reach these members.
