With thirteen years of experience working as a psychiatric mental health registered nurse (RN), and a current PMHNP-BC certification with a solid educational background, I am a qualified candidate for the Psychiatric Mental Health position in a company. PROFESSIONAL / CLINICAL WORK EXPERIENCE:

1)QCI of Southern Maryland (Currently employed as PMHNP-BC)

• Establishing and/or maintaining appropriate standards for treatment, including therapeutic modalities and prescribing practices.

• Attend to medication aspects of quality management.

• Collaborate with other medical professionals, including a review of diagnostic

formulations and documentation of physical examinations.

• Provide OP and Mobile Treatment psychiatric-related services including psychiatric evaluations and assessments, prescribe, modify, and monitor all medications as necessary. Conduct Initial Psychiatric Evaluations within required COMAR regulation guidelines, including.

• Formulate and document the psychiatric diagnosis and the rationale for the diagnosis or affirm a psychiatric diagnosis that has been entered in the individual's medical record.

• Assess the individual's level of functioning and availability of family and other social

supports.

• Order and monitor tests at medically recommended intervals and document the results in the individual's medical record.

•Develop Individual Treatment Plans (ITPs). Participate in the recommended discharge of patients, including signing the Discharge Plan.

• Conduct initial nursing assessment when first assigned a patient. Discuss with the patient the nature and boundaries of the therapeutic relationship, and patient goals.

•Do an ongoing evaluation of the effectiveness of all interventions and make revisions as needed.

• Assist patient in obtaining any prescribed psychiatric medications by coordinating with CSS regarding insurance and applying to the CSA and pharmaceutical companies as needed.

•Administer psychotropic medications (Injectable as Abilify, Vivitrol, Invega, Aristada, Haldol, and PO medications) per the doctor/NP's orders in a timely fashion.

•Document all doctor or NP orders (including any changes in meds, orders for samples, orders for pharmaceutical companies, etc.) as Medication Adjustment notes.

• Educate patient, and family when indicated, regarding medications, side effects, and potential adverse events and develop a plan to assist patient in taking medications as prescribed and reporting any problems.

• Assist the patient in obtaining and seeing a primary care provider (PCP) and encourage the patient to follow through with somatic recommendations. Educate the patient about any somatic conditions and recommended treatments, including lifestyle changes and nutrition.

• Record vital signs and perform a Mental Status Exam. Assess for any adverse effects of meds, any safety issues, symptoms of mental illness or drug use, changes in eating or sleeping habits, or other behavioral changes.

• Participate actively as a member of the treatment team. Review and sign treatment plans within 24 hours or per COMAR guidelines.

• Coordinate with administrative staff to ensure records are available from all pertinent sources, including hospitals, doctors' offices and other agencies and needed information is exchanged with appropriate release from the patient.

• Secure pre-authorizations for patients at the request of the doctor/NP when needed

• Take personal and professional responsibility by participating in life-long learning through attendance at professional/educational seminars, home study, participation at pharmaceutical presentations, etc.

• In the RMA/CMA’s absence- to be a liaison between the pharmaceutical reps and the doctors/NPs and to assist in arranging educational opportunities for staff.

• Assure that medications are delivered to the patients in a timely fashion or that meds are discontinued as needed.

• Assist patients with reminds of doctor’s appointments and transport them to sessions with primary care or other physicians as needed and as team agrees
