PROFILE SUMMARY
SOCIAL WORK -CLINICAL SETTING, HOME HEALTH CARE, HOSPICE: Perform assessments, counseling, education, and any other needed social services for patients and families. DISCHARGE PLANNING/HOSPITAL SETTING: Conduct assessments, referrals for counseling, equipment, hospice and other services, and nursing home placements. Provide community service and education. PSYCHOSOCIAL ADJUSTMENT ISSUES IN THE RETIREMENT COMMUNITY: Perform assessments, care plans, interventions to establish and maintain relationships with residents and their families.
PROGRAM MANAGEMENT - Plan, organize, and manage patient education programs: coordinate overall activities and classes. Set-up continuing education for health professionals and serve as an educational resource in both hospital and community settings. Supervise patient education staff. Serve as liaison and resource for community agencies. Assess, counsel, and promote self-advocacy among patients and staff.
HEALTH EDUCATION- Plan and present health education in such diverse areas as family planning, school health, lead poisoning, HBP control, diabetes, and cancer. Provide counseling services on both an individual and group basis.
RESEARCH - Conduct interviews and surveys designed to access and increase public knowledge of available community resources and facilities. Work closely with statewide project directors, local medical directors, and community interest groups to effectively coordinate and accomplish research objectives. Collect relevant data for use in evaluation and presentation of statewide health projects. Assist in preparation of budgets, statistical analysis, maps, charts, and other visual studies. Develop and implement programs to determine effective methods for communicating health information to rural communities.
