Experienced Social Services Case Management Director with expertise in collaborating with and care coordination services provisions that relate to Program Management. Assistant Vice President 2/2023-Present

Ensure that all direct and subordinate staff complies with CAMBA’s and funders’ polices and rules.

Oversee all program operations and manage direct reporting program staff and their subordinates.

Develop and implement Quality Assurance measures to ensure quality service delivery to clients.

Ensure that all client files and program files are kept in compliance with CAMBA's and funder's standards.

Direct, coordinate, supervise and document programmatic activities facilitated by direct reporting staff.

Initiate and proactively create solutions to address gaps in shelter services or staff performance (i.e., basic computer training use of technology).

Address and implement resolutions to client/staff complaints in a timely manner.

Develop and implement crisis prevention and/or intervention strategies to ensure a safe and secure environment.

Review and sign time sheets.

Supervise, review and approve staff and subordinates’ training, vacation and leave.

Case Manager 8/2021- 2/2023

CAMBA Queens,/ New York

Under supervision of the Director of Social Services, responsibility for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program.

Provides case management and on site crisis intervention

· Completes required service plans.

· Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord/tenant relations, as necessary

· Maintains appropriate documentation of all tenant contacts.

· Documents monthly progress notes in the participants’ chart

· Conducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services

· May escort participants to social services agencies, as requested

· Reports any alleged child neglect or abuse to the Director of Social Services

Case Manager 12/ 2020-8/2021

Trained and supervised incoming Case Managers and Residential Aides in according with federal and state policies including Case Management duties including Department of Homeless Services Procedures, Incident Follow ups and Cares Documentation.

Supervise incident follow-up procedure to ensure an client concerns are addressed.

Exceed Case Manager’s job requirements including working and completing assigned tasks for the mandatory assigned 25 homeless shelter families caseload.

Formulate plans and prepare clients for permanent housing placement; Engage individuals requiring social service assistance for insurance, benefits/entitlements; Interview and evaluate clients, formulate Independent Living Plans and goals, locate and refer to appropriate community resources for clients. Ensure ACS and preventative services recommendations are updated, monitor and assist newborns and mentally challenge individuals as needed.
