RESPONSIBILITIES

1. Greets patient on arrival in the pre-operative area.

2. Identifies the patient, applies arm band and initaites nursing documentation in the patient's medical record.

3. Assures belongings are properly labeled.

4. Questions patient on allergies, last intake of food or drink, and charts responses.

5. Verifies operative consent for correct procedure, doctor's name, patient's signature, and date and time.

6. Verifies chart has completed history and physical.

7. Takes vital signs and charts on arrival and as necessary.

8. Completes pre-operative check list and signs his/her name.

9. Verifies chart for appropriate laboratory work and notifies physician of any abnormal value, which also reflect in the medical record documentation.

10. Verifies physian's orders and performs procedures as ordered.

11. Conducts the pre-operative patient assessment according to facility policy and in accordance with anesthia guidelines.

Recovery Room Nurse

1. Receives report from the anesthiologist and circulating nurse and identifies patient.

2. Takes vital signs when the patient arrives in recovery and every 15 minutes therafter unless otherwise ordered.

3. charts patient's color, level of consciousness, condition of dressings and character of respirations on arrival and every 30 minutes while patient is in recovery.

4. Verifies physian's orders, administers medications, and performs procedures as ordered.

5. Reviews the discharge instructions to the appropriate person.
