Major Responsibilities

Used my interest in reviewing updates in medicine and in nursing to enhance my resources

I referred to critical decision making in collaboration with the team. Made rounds with CEO staff of the medical center in N.C. insurance group to assess the cost of patient care, discharge days, patient care needs and re-admissions. I began my role as a nurse in the critical care setting. I was charge nurse in the Neurosurgery unit and assisted in the various units such as Trauma, Med Surg, and Cardiac care.

Throughout my career I have used some aspect of critical care in my jobs. I was employed in NC at Chapel Hill as a Transplant Coordinator. I worked with adults and pediatrics in the Heart Transplant program I made rounds daily with the physicians associated with the heart unit and the physicians with medical offices who specialized in

Heart Failure

I was responsible for the pre- and post-transplant patients. I was required to know the transplant meds, signs and symptoms of rejection, tests, and lab results

On call every other weekend.

I attended symposiums and traveled with the team of physicians and nurses.

We were on a panel team of discussion with other medical center personnel. In review my jobs involved multi-tasking, research, patient care, implementing resources, collaboration, families, communities, teaching, education, and a registered nurse that cares about her job.

My Complex Case Manager Role:

Under the supervision of the Complex Case Management Program Nurse Manager, the Registered Nurse (RN) Complex Case Manager (CCM) is responsible for intake and providing case management services for the medically and/or socially complex patients who are medically homed within UW Health. The patient population covered will include significantly complex medical conditions, and/or social-economic and mental health co- morbidities. The goal of the program will be to assist these patients to achieve optimal health and/or independence in managing their care. To achieve this goal the manager will demonstrate and apply knowledge of the philosophy/principles of comprehensive case management, patient- centered, culturally sensitive care coordination and management of complex patients.

The case manager will be responsible to develop plans for patient and family self-care competence, including motivational assessment, assessing for desired level of involvement and coaching for adherence to care plan. CCM will provide nursing assessment, create, and monitor patient/family care plans, including end of life planning. The primary contact with the patient, family and other involved care providers will be by telephone, and electronic medical record (EMR).

The case manager promotes knowledge of this program throughout UW Health, additional hospitals, and programs where UW Health medically homed patients live and receive care.
