PROFESSIONAL EXPERIENCE:

Outpatient Medical Social Worker - Veterans Health Administration (40 hours a week)

February 2023 – Present

Provide outpatient support and assistance to Veteran’s in need of a variety of social services. Asses risk, report abuse, and collaborate with a multidisciplinary team. Refer Veterans to a variety of VA and community programs. Connect Veterans to food, housing, and financial resources and provide referrals to Medicare and Medi-Cal. Conduct psychosocial assessments for transplant candidates and VA programs. Work with Veterans and families who are experiencing emotional, medical, and social problems utilizing individual, group, and family counseling skills. Provide excellent customer service skills by handling conflict and problems in dealing with the customers constructively and appropriately. Uses the Veterans Health Information and Technology Architecture to access information and upload documentation into the Medic Center Computer System including the Computerized Patient Record System. Coordinate care of Veterans and families who live in different states and cities. Provide supportive psychotherapy services for caregivers in the Caregiver Support Program. Perform presentations to incoming medical residents regarding the role of outpatient social workers.

Inpatient Acute Medicine/Surgery Social Worker - Veterans Health Administration (40 hours a week) August 2022-February 2023

Complete psychosocial assessments and assess for high-risk situations and behaviors. Communicate with multi-disciplinary team about these issues and complete appropriate documentation. Assist Veterans’ in educating and completing advance directives. Assist and counsel Veterans and families in adjusting to psychosocial issues related to their disabilities/illness and/or death/dying issues. Educate Veterans and families about VA and community resources. Conduct treatment planning, participates in interdisciplinary collaboration, and provides consultation. Provide coordination services, care management, and aftercare follow-up, depending on the work assignment. Screen for substance abuse, tobacco use, depression and PTSD and, depending upon assignment, provides treatment or referral services. Order patient consults in selected program areas. Responsible for identifying patient problems, which require consultation with medical center staff of other disciplines. Knowledge of disease processes, surgical procedures, medical terminology, treatment and the impact of the Veterans/families lifestyle.

Community Resource and Referral Center - Veterans Health Administration (20 hours a week)

September 2021 – May 2022

Identify and engage homeless or at risk Veterans and provide wide range of services including assessments, referrals, short-term case management, advocacy, service coordination, and linkage with the VA and community partners. Work with multi-disciplinary team including Peer Support Specialists, RN, social workers, community partners, transitional housing programs, HUD-VASH, and various multi-agency services. Promote and provide referrals to both short-term and long-term permanent housing in addition to mental health care, access to VA benefits, primary care and other resources. Demonstrate knowledge of the medical, mental health, and social needs of Veterans who are homeless or at risk of homelessness. Participate in team conferences and trainings to promote professional development and coordination of service delivery. Devised various macro projects including the design and modeling of a physical and digital map depicting transitional housing programs and SSVF locations. In addition, worked on asset mapping project used to identify transitional housing program’s existing resources.

Korean Community Services (30 hours a week) June 2020 – August 2021

Worked with diverse age groups including children, adolescents, adults and older adults. Provided case management services, conducted home visits and provided resources and referrals to various programs and multi-agency services. Provided psychotherapy to children and adults to effectively implement appropriate interventions. Conducted biweekly English classes for older adults and implemented Substance Use Disorder and Adolescent Mental Health Trainings twice a month.

UCLA Research Assistant (30 hours a week) October 2019 – March 2020

Utilized Elan software to transcribe audio recordings of interviews using conventional transcription system for conversation analysis. Programmed using Python to align audio with individual words from interviews. Utilized Praat software to align interview to transcribed text. Analyze data and trends from interview recordings and report findings to lead professor. Created instruction manuals detailing how to transcribe with Elan software for future research assistants. Multi-tasked, worked independently, and improved time management skills as an RA.
