PROFILE
I have an unique ability to really see and connect with the individuals who I’m interacting with. The individual may be experiencing homelessness or is in active addiction, but I am still able to get my message of Support and resources to them. I can be training a provider or a med- student on how to work effectively with an individual who is in active addiction it can be as if I am training a family member. Anyone who interacts with me can feel the respect and value that I see in them. This is what I know makes me an affective Peer Support Specialist.
EXPERIENCE
Healthcare Coordinator Volunteers of America of Western Washington 06/2023-present
Oversight of all clients in the DS program health and medical needs. Obtain and maintain all medical consents to provide healthcare treatment. Oversight and management of program database, tracking all clients medical records. Receive and communicate all provider care instructions to Program Managers and there teams.
Schedule all healthcare appointments, including annual physicals, annual dental, Specialty visits, follow up appointments, therapy and psychoactive medication reviews. Primary point of contact for Nurse Delegator and responsible for communicating any changes to delegated tasks or medication.
Monitor employee NAR status and maintain current records on PHS to Comply with Nurse delegate laws
Attend Assessment meetings and contribute to the creation of IISP, Medical Device Plans, and relevant Protocols.
Coordinate with training department to instruct new employees on the use of medical devices or changes in medical treatment plans. Complete all client’s LTC eligibility Reviews.
Review and Audit routine medication checks completed by House Coordinators and Program managers.
Review Mars at the end of the month to check for accuracy and follow up with the Program managers and address any discrepancies.
Attend medical appointments as needed.
Coordinate on incident reports follow up where further medical follow up is required. All duties are assigned by the Regional Director.
