Kaiser Permanente of Washington, Renton, WA 8/2022 – 02/2023

Hematology / Oncology Clinical Pharmacy Specialist

Implemented a program with the goals of improving three key components of pharmaceutical care: (1) Patient satisfaction scores, (2) Provider adherence and utilization of organizational oncology pathways, evidence- and guideline-based medicine, and (3) Cost containment. As the oncology subject matter expert for the Prior Authorization Help Desk team, managed requests for pharmaceuticals used to treat cancer (biological agents, growth factors, immunotherapies, antineoplastics, etc.). Reviewed patient-specific information including pathology reports, cytometry, and molecular testing, next generation sequencing, etc., to confirm diagnosis, age, performance status, co-morbidities, stage, grade, and other information necessary to evaluate appropriateness of therapy, confirm therapy is concordant with recommended sequence outlined in the pathway, and/or national guidelines, FDA approved- and/or off-label indications, as appropriate, and with consideration of the patients’ insurance provider (Medicare Part B, Medicare Part D, and commercial insurance). If/when discordance, and/or a therapeutically equivalent, yet more cost-effective treatment is identified, or if a less costly, formulary-preferred biosimilar is available, engaging the prescriber in a peer-to-peer discussion is implemented and alternatives are shared.
