Rocksure is a mining services contractor with a workforce of over 700 workers across 3 sites and about 55 fleet of heavy-duty mobile equipment which are used for contract mining services in Perseus Mining Ghana Limited in Ayanfuri and Ghana Manganese Company Limited in the Central region and Asanko Gold Mine in Ashanti region of Ghana respectively on a 12-hour-3 shifts system. Rocksure International delivers Drilling, Blasting, Mining and Haulage of earth material from the Pits to Rom pad or Waste Dumps of average distances 15km, 4km and 2km respectively. The prominent hazards / risks include uneven running floor conditions, tyre failures, fatigue driving, pit wall failures, interaction of heavy-duty mobile equipment with humans, hazardous interaction of heavy-duty mobile equipment with light duty mobile equipment, management of explosives etc.

The following are elements of my role and duties:

• Leading risk assessments (job safety analysis) using companies risk management framework matrix and prioritising significant risks and developing mitigation programs against the key risk identified.

Conducting site monitoring of unsafe actions and conditions created due to mining activities and ensuring corrective and prevent action plans are developed and implemented to prevent occupational incidents.

Leading incident investigations and ensuring that corrective and preventive actions determined for each identified root cause are implemented as well as following up on outstanding measures where applicable.

Leading the arrangements for employee consultation involvement, and participation as part of the health and safety management system arrangement.

Conducting training using developed safe operation procedures (SOPs) and safe working procedures

(SWPs) for awareness creation and for competency training of workers for specialised work activities.

Conducting surveys and developing traffic management plans for new operational activities and ensuring all essential safety information and warning signage are provided.

- I led the investigation to determine the root causes for tyre failure and determined control measures which has helped reduce the rate of tyre failure incidents.

Vodafone Ghana commenced health safety and wellbeing programme in 2009 to address high incident frequency rate averaging about 9 fatalities per year as well as 23 high potential incidents per year while operating a fleet size of about 700 vehicles. The five key occupational health and safety risks included working on structures as high as 100m and in permit required confined spaces and extensive work with or on electrically powered installations and occupational road risks. Amongst the programs and initiatives that were deployed to proactively manage the occupational road risk and other key risks of the organization, I led the introduction of controls including contractor engagement and management, driver behaviour modelling program and employee refresher training programs covering all teams, maintaining awareness among employees and contractors via campaigns and awareness trainings, implementation of network design standards to improve the safety of climbers, monitoring of managed service contractors to ensure conformance to Vodafone requirements which in many ways brought huge benefits and results to the company. The following are some of the key functions:

- I implemented Health Safety & Wellbeing (HSW) programs that progressively reduced fatal incidents by 57% in the first year, 14% in the second year and 29% in the third year.

- I improved the HS&W culture by conducting project specific trainings for employees and contractors and maintaining records of participation.

- Supported the Supplier Chain Management team to win the African Chartered Institute of Purchasing & Supply (CIPS) award (Health & Safety Category) for year 2011 organised in South Africa.

to Guinness Ghana Breweries Limited, Accra (Ghana) 01/03/2010

Guinness Ghana Breweries is a food and beverage manufacturer of offering about eight (8) different products to the Ghanaian market. Their raw material and energy needs includes: glass bottles, water, cereals, caustic and electricity, diesel and residual fuel oil, as the main inputs whiles generating some significant by-products like chaff and environmentally degrading effluent streams. The Achimota site had 3 processing lines, run on 24/7 basis and operated by 3 shift system each consisting of 100 employees and contract staff and office staff population of about 150 on usual working day. As glass bottles were filled with finished beverages under gas pressure, explosion of same led to frequent severe injury rates of at least 16 lost time accidents per year in addition to over 33 medically treated cases per year which are also attributable to the same cause. The processing plant had issues with heat accumulation, ventilation and high humidity which were a significant challenge for employee’s comfort.

I was responsible for the establishing and maintaining safety management systems and also managed the environmental aspects aimed at reducing the risk of injury to employees and potential environmental pollution incidents at the Achimota Brewery site. To ensure the company meets environmental standards I liaised with the all relevant regulatory to ensure compliance to statutory and other requirements through reporting per the required frequencies and timelines to the relevant agencies. My duties included:

Coordinated and facilitated Crisis Management activities and trained the organization’s Crisis Management Team including desktop simulations, real-life business continuity and recovery exercises as well as maintained War Room and relevant equipment in readiness.

Budgeted for annual health safety and environmental programs and tracked same in with procurement department and financial to control expenditures

Provided professional advice to senior management on statutory/regulatory requirements and how to meet those requirements by keeping a summary of applicable legislations and tracking compliance at established frequency.

Liaised with all applicable regulatory agencies (both national and local community) to ensure their requirements are met thereby eliminating the risk of corporate fines or infraction notices.

Developed targeted standard procedures to address identified operational hazards & risks and monitored to ensure compliance of same after training on those procedures.

Achievements - I developed glass handling procedures wit hazard area classifications and implemented strict rules to reduce contact with glass on the process plant which drove down the 16 LTIs to 3 in less than 3 years.

- Also, I initiated the process to end inadequate ventilation in the process plant floor by engaging the service of ventilation consultant and brokered an agreement to conduct a preliminary survey, waving the initial cost of consultation, for the installation of heat exhaustion and ventilation fans and thereby reducing the heat stress and the associated discomfort of workers in the area.

- I established permit to work systems that covers all areas of high hazard high risk operations in the processing plant and trained all technicians, team leader and supervisors on their roles in acquiring permits to work as well as general employees’ roles and responsibilities under permit to work system.

- Set up crisis management room (CMR) with all relevant equipment, tools and materials in readiness for crisis management activities for the entire Guinness Ghana Limited on the Achimota site.

- Developed simulation exercises and conducted 2 crisis simulation drills annually for two years in the Guinness Ghana Achimota site as well as emergency drills

- I achieved zero enforcement action record during my entire tenure from 2007 to 2010 in Guinness Ghana Limited and ensured timely completion of license to operate (LTO) audits on yearly basis.
