Assist the case management team with facilitating the safe and timely transition of patients from hospitals to lower levels of care and all related out patient follow up needs. Ensure patient-specific continuity of care during their transition to a lower level of care. Interview patients and/ or families to assist the Case Manager with the identification of potential barriers to discharge, including but not limited to, financial, emotional, inadequate support systems, and/ or other patient-specific barriers.
