Assists the member in identifying and developing natural supports (family, friends, and other community members) and resources.

Develops a member-centered action plan with goals and next steps for the management of their medical conditions and substance use. The action plan is reviewed with each member contact.

Performs risk stratification per program guidelines in order to determine a contact schedule for the member.

Responds to Significant Event alerts regarding a member’s hospitalization or ED visit according to guidelines established. Conducts an in person visit in facility or at home and reviews conditions precipitating event, coordinates referral to substance use treatment, and refers to additional community supports as appropriate. Empowers member to follow up with medical appointments and adhere to medical plan of care.

Coordinates referrals to substance use treatment providers for inpatient detox, intensive out-patient treatment, out-patient treatment, inpatient rehab facilities and community based recovery and support groups as appropriate.

Supports and empowers patient in recovery for signs of relapse and referrals to community supports. Empowers member to engage with natural supports in addition to the community.

Conferences with team on complex cases and performs Root Cause Analysis for hospital admissions and recovery relapse.
