Enter demographic data, contacts made, and other required data for all newly assigned clients into the electronic record system in a timely manner. Facilitate the development of an individualized Service/Treatment Plan. Work with child, youth and family teams to develop a crisis plan to ensure the safety of the child, family, and community. Utilize approved skills curriculum with child, youth and family to help improve behavior. Coordinate and facilitate ongoing child, youth and family team meetings. Provide 18-25+ hours of billable services per week; the behavioral health professional should expect 25 to 35 clients on their caseload depending on number of referrals. Take initiative to request new client referrals as caseload numbers drop due to discharges and/or upcoming discharges. Meet face-to-face with clients in their home, school, office and community for the provision of all services. Initially assess and reassess (every 90 days or sooner if required for reassessment) the needs of the client by completing a CANS/ANSA Assessment. Meet with the client, family and all team members, and modify the Service/Treatment Plan to reflect the changes and/or progress made. Keeping the family and team members informed on progress toward achieving Service/Treatment Plan goals. Motivate and support family involvement. Assist client and family in obtaining any financial assistance or services for which they may be eligible under federal, state, and local regulations. Maintain accurate and concise documentation of all services. Contact and collaborate with client, family, and community partners. Provide or secure support and crisis/emergency services for the child, youth and family by phone-contact or in-person. Responsible for working with adolescents dangerous/aggressive youth 10-17years of age that are on Juvenile probation or involved with the court in Harris County only. Worked with adults on parole with mental disorders, case management, working as a guardian. Conducting face-to-face visits to the schools and homes in Harris county monthly, probation meetings monthly. Conduct family counseling and individual counseling with adolescents. Serve as a resource to obtain services for youth and their families, arrange appointments, worked with runaways providing crisis counseling, and long-term counseling if needed, assisting parents with arranging voluntary placement for kids who do not follow rules at home or on probation. I keep track of each chart by making sure every document in the chart is in the correct order. Working with truant kids on probation, arranging and connecting as a team to eliminate the course of truancy from continuing. Have 7yrs of engaging with primary caregiver and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement also implementing a problem conceptualization, treatment planning, intervention implementation. Conduct face-to-face home visits and at the school to provide counseling services. Responsible for giving families referrals using wraparound. Have over 7 years of Counseling and providing case management to dangerous/aggressive youth and families in crisis for example parent/child conflicts, runaway, truancy, aggressive behavior etc. Worked in Harris County Leadership Academy 2009-2010 when MHMRA was contracted with detention centers.
