Family Support Services of North Florida, Jacksonville, Florida (December 2020-Present)

Housing and Placement Specialist

Family Support Services is the lead agency for community-based care services. It assists and supports parents in their role as caregivers.

The Housing and Support Specialist is responsible for providing case management services for young adults in the PESS program, OCS services for all out of county youth placed in Duval County and securing housing for young adults in Extended Foster Care program. This position also acts as a housing resource for PESS clients by collaborating with community members to create a network of providers and/or landlords willing to assist with clients that might not meet standard apartment income requirements.

This position is designed to assist the assigned case manager with housing options for the young adults participating in the EFC and PESS programs. This worker will need to be the single point of contact for all housing needs; completing the Shared Living Plans and Living Assessment forms for each provider and young adult in the program, completing change forms, updating FSFN, and any other applicable systems as needed.

PESS specific responsibilities include the examination of the young adult performance in the PESS program and ensuring the young adult maintains their qualifying activity. The Housing and Support Specialist is also responsible for completing monthly home visits for all OCS cases assigned to FSS. The Specialist may also be required to perform home visits for EFC young adults if needed.

National Youth Advocate Program (NYAP) - Jacksonville, FL (July 2019 - January 2020)

National Youth Advocate Program (NYAP) is a private, non-profit agency with a non-negotiable commitment to do what is best for children, youth, and their families.
