I recently became aware of a therapist position within your agency. Having over 15 years as a social service worker, I have been exposed to working in a multitude of areas to include skilled nursing facilities, psychiatric hospital settings, and therapy settings. Having graduated with a master’s degree in advanced clinical social work I took it upon myself to experience clinical internships and employment opportunities in a multitude of environments to experience various social work/therapy trajectories. My experiences have exposed me to varied positions that has equipped me to work on interdisciplinarian and multi disciplinarian teams in Geripsych environments, as well as skilled nursing facilities to collaborate and provide exceptional care and advocacy for patients. I am well versed at the facilitation of medical /behavior health intake, assessment building, treatment planning, utilization and review, discharge planning and working with various assessments. I am well versed with Medicare, Medicaid, commercial insurances, and determining and facilitating presumptive eligibility for uninsured Medicaid/Medicare patients. I understand the importance of being an advocate to insure the most effective coverage for patients, as well as the importance of proper care planning at discharge to eliminate unnecessary readmissions. Additionally, I have experience with obtaining prior authorization for clinical therapy services, utilization and review, and insurance brokerage services. I have been exposed to curriculum that will further assists me in working in the field of social work therapy providing advocacy, intervention, and education with special populations while aiding in combatting holistic issues utilizing the ecological, systems theory framework, and other varied theoretical frameworks to empower clients, patients, and families. Work independently and collaboratively as part of a care team by collaborating with other team members as it relates to clinical service model, facilitate individual and group therapy sessions that builds recovery skills with the utilization of outlined curriculum, complete intake paperwork including the completion of Biopsychosocial, and ASAM.
