WORK HISTORY:

7/1/2015 to 9/1/2020 ~ NHC Home Health Care, Hendersonville, TN. – PRN

This was a PRN job in which I usually worked approximately 4-6 days per month performing scheduled and on call visits to patients’ home. Duties included physical care as well as admitting patients to the service for care. RN also assisted with ordering and scheduling services such as PT, OT, ST, as well as appointments with their PCP and specialists the patient may need.

7/2/2018 to 1/5/2023 ~ BCBST, Chattanooga, TN. - Remote

Nurse case manager for the Intellectual Disabled and Mentally Challenged population. This included assisting with ordering medical equipment, assuring a safe environment, assist with PCP visits, and assisted that preventative measures were met to include HEDIS measurements. Performed documentation of member assessment each month to include the above and any new conditions, medications, and s/s.g

9/2014 to 6/2018 ~ Humana, St. Petersburg, Fla. - Remote

Nurse Case Manager for members who had Chronic Disease Conditions. NCM monitored, assessed, and provided education and encouragement on an ongoing basis to give them the members the knowledge to allow them to monitor and care for their chronic conditions in their own home. This allowed members to avoid accessive ER visits, hospitalizations, and MD visits.

12/2012 to 9/2014 ~ Donelson Care & Rehabilitation, Donelson, TN.

This was a long term care facility as well as rehabilitation for patients with short term rehab needs. My role included Asst. Director of Nursing and also assisted with MDS processes.

Role of ADON included checking documentation on patients and monitoring the care for long term patients as well as rehab patients. Assured that the care as well as the documentation for our patients met the states requirements.

8/2011 to 12/2012 ~ Williamson Medical Center, Franklin, TN

Critical Care Unit where duties included acute care and duties for critically ill patients including medical, trauma, surgical, cardiac, and orthopedic patients. Also assisted MD's with performing such procedures as placing central lines, chest tubes, etc. RN was responsible for assuring that MD was informed regarding patients, their needs, desires, and condition.

7/2005 to 8/2011 ~ Ashland City Medical Center, Ashland City,TN

Duties included being a RN Charge Nurse for a 16 bed medical, surgical, and rehab unit. RN Charge Nurse over RN’s, LPN’s, and CNA’s.
