Collect the information from the community.

Report the case to the S.O of WHO-EPI

Frequent site visit on daily bases to different villages.

Take randomly cluster of the concerned villages for

REN

Identify Miss HH for polio eradication initiative.

Identify mobile children or migrated children during

polic round.

Bring awareness in the community about polio

eradication initiative.

Entry the data into the data base.

Share daily, weekly report to AHCSO.
