In a group house setting I was responsible for the supervisor of mentally and physically disabled individuals. Making sure they were in a peaceful and safe environment at all times. I did light house work and prepare meals. Making sure that the client were taking medication and cleaning up behind themselves as much as possible in their homes. Reporting and documenting all activities to the managers of the house and making sure all necessary updates are given to incoming staff. Transporting client in my personal car to and from events and appointments

Taking vitals and making sure client was oriented and sure they are doing well. Doing adl’s that would include taking baths/shower making bed and cleaning client area and reporting any activities that may be strange and unusual to family and staff. Making sure clients take medication and eat food. Preparing possible 2 meals per shift and changing the client and or assist them to the bathroom. Constant communication with the client or family as needed. Making sure the client was safe and secure at all times. On some occasions the client did need assistance going to different appointments and I had to drive them using their vehicle. At the end of shift I would make a record in the app of the different activities that we covered and what may have been refused.

I worked the front office area with the staff of ladies helping prepare documents for the next days shift load. This included scheduling with other specialty clinics and dr.’s offices. Checking for insurance coverage and if they need referrals, making sure assigned doctors documents are uploaded in the chart that would be needed for appointments as well as out of office documents, scanning in faxing over paperwork to the appropriate areas, make sure any imaging that needed is in the chart, answering the phone calls and scheduling appointments, retrieving archived records, having documents notarized for the office and working on Athena system with Word, Windows and Excel
