·Responsible for assessing level of care needed in the facility. Discharge planning from admission so that residents can move to lower level of care rather than stay in skilled nursing long term. Many of the residents had been custodial for long periods of time and I was able to transition them to Assisted living or Board and Care with as many resources that were available such as Waiver programs and financial assistance. The goal is to improve quality of life and open up beds for skilled patients improving business goals. The facility also needed a Social Worker for their SNF section and I was able to use those skills from prior position to take responsibility for that unit.
