Legacy Behavioral Health Services of South GA: Valdosta, GA. March 2001 to November 2022. Work 40+ hrs per week. Most recent duties include: Lead Treatment Provider for Lowndes Co Accountability Court (April 2016 until November 2022) – coordinating and providing group sessions for program participants (Adult felony convictions – age range 17+) that suffer from substance abuse and/or mental health issues. Focal point on Honesty, Accountability and Responsibility; worked with clients on decreasing criminal mind-frame to improve functioning in society and meet legal requirements. Managed caseload of 30 to 50 program participants - completed clinical documentation in individual charts including daily group/individual notes, bio-psychosocial, ANSA, ASAM level of care placement, CSSRS (suicide risk assessment) and pertinent medical information needed for holistic treatment. Completed individualized treatment plans, with client centered goals targeting deficits in living, learning, working and social environments. Provided individual therapy sessions and group counseling to focus on program curriculum as well as identified client need(s). Emphasis placed on building rapport with the clients and forming trust within therapeutic relationship. Providing of treatment to program participants involved in the criminal justice system, under curriculums/modality set by CACJ (Council of Accountability Court Judges). Collaborate with and coordinate continuation of care with team members (including program coordinator, Superior Court Judge, Assistant District Attorney, Public Defender, Case Manager and/or other court personnel or clinical staff needed for implementation of legal orders and best care practices). Crisis/Safety planning including evaluation of client’s risk of harm to self/others or presenting with overtly psychotic symptoms/could pose danger if untreated, referrals to residential and/or detoxification programs. Short-term and Long-Term referrals placed to treatment facilities based on legal issues, client motivation and future goals/plans.

Previous Responsibilities: Supervisor of Community-based services (CSI – Community Support Individual) 2-4 case managers (2013 to early 2015) Completed in-depth client assessments to formulate treatments that include non-traditional type of services (Home visits, Community activities) ie.. medication monitoring, mental health education, applications/referrals for ADL’s, housing supports, and connection to resources for client’s improved level of functioning. Assigned/tracked cases that was demographically friendly for improved production and cost-effectiveness. Supervised staff for approx 3 mths that interacted with juvenile population requiring more intensive, community based services. Provided information/documentation for school teachers/counselors for developing IEP/Treatment plans. Evaluated behavior modification trials for Conduct Disorder / Oppositional Defiant Disorder, ADD/ ADHD diagnosis. Collaborated and discussed efforts with family members involved in treatment for behavioral contract/token-reward system and educated on how to utilize for different responses.

Intake/Assessment Clinician (Adult and Adolescent clients) - Evaluate, probe/explore and collaborate with client to complete detailed documentation for opening of chart for community mental health clinic for ongoing therapeutic services.

Crisis Response Unit (CRU) (February 2011 – April 2012) Hospital Emergency Room based services; after hours assessment/referrals/placement for individuals presenting to ER for mental health crisis. Served adult and child population in ER for Valdosta, Brooks, Cook, Berrien and Lanier counties. Developed relationship with individuals in crisis situations to evaluate and make determination for voluntarily or involuntarily admission into inpatient psychiatric facility. Collaborated with hospital admission staff to facilitate acceptance for adult and adolescent clients in need of stability.
