WORK EXPERIENCE – CLINICAL

March 2021-Presently Appletree Corporation Telemedicine Assistant I work on a full-time basis an average of 40 hours/week. I conduct full examination, take thorough medical history, consent the patient to the telemedicine program and discuss the findings with the physician via videoconference. I alert the physician to points of concern and direct his/her attention to additional tests warranted in selected situations

May 2018 – January 2021 Telemedicine Assistant at Long Sault walk-in clinic, rural Ontario

From May 2018 to the present time, I am selected to work as a telemedicine assistant at Appletree Medical Group in rural Long Sault Ontario, a community of 8000 people. I work on a full-time basis an average of 40 hours/week. I conduct full examination, take thorough medical history, consent the patient to the telemedicine program and discuss the findings with the physician via videoconference. I alert the physician to points of concern and direct his/her attention to additional tests warranted in selected situations. For example, I alerted the physician to the possibility of severe pneumonia in a child with intercostal retractions and rapid respiratory rate which warranted referral to the Emergency Department. Furthermore, I notify family doctors of patients seen at the clinic for continuity of care following their visits to the Telemedicine Clinic. I perform daily full assessments of patients (adults and pediatrics) in the following manner depending on the patients complaints: I perform neurological assessment of power, sensation, coordination, cranial nerves, Kernig and Brudzinski's signs; I assess children for Kawasaki disease, ear, nose and throat problems ( I also assess adult patients for ear, nose and throat problems as well); I perform abdominal examination and assess for hepatosplenomegaly, I conduct chest examination for assessment of lung fibrosis and I assess for infections such as pneumonia and acute bronchitis, in addition to taking throat swabs, urine dip testing for urinary tract infections and instructing the patients to perform vaginal swabs when presenting with vaginal secretions to test for infections such as trichomonas vaginalis and vaginal candidiasis. I send specimens to the local laboratory for testing. I examine patients with skin conditions ranging from simple infections such as impetigo to suspected skin cancers such as basal and squamous cell carcinomas. I also contact specialists for patients who need referral. Moreover, I examine patients visiting the clinic with teeth and gum infections and report gum inflammatory conditions to the physician via videoconference for appropriate treatment; and I advise patients to follow up with their dentists. Part of my daily work includes alerting physicians to possible drug-drug interactions e.g. in patients on birth control pills who were to be prescribed antibiotics, drug interactions involving QT prolongation and drug interactions reported by the pharmacist.

I communicate regularly with the Public Health Agency of Canada for following up with patients with suspected Lyme disease given that Lyme disease is very common in the area of Long Sault Ontario.

My daily routine with all patients visiting the clinic includes taking vital signs for patients who come to the clinic as a preliminary assessment of their general condition, filling-out forms for patients on behalf of the physicians with regard to injuries in the workplace (the WSIB form), sick notes, notes for schools to explain why children cannot attend examinations and forms for work accommodations for patients who need to telework for their medical conditions. I also assess patients with low back pain, ankle sprain, Achilles tendon pain, plantar fasciitis, hip, elbow and neck pain and test for the range of movement and perform specific tests as Thompson test for Achilles tendon rupture, in addition to specifying the required x-ray views for patients with suspected bone fractures. Finally, I assess patients prior to their travel to endemic areas for their vaccination needs against malaria, Japanese encephalitis, and Typhoid.
