Assessment, following through with provider orders within scope of practice, and re-evaluation of interventions and patient outcomes. Responsible in disposition and placement for patient in regards to admissions, transfers, and discharges. Working with insurance companies to provide clinical to insurance company representatives to ensure appropriate indications and accommodations for admissions, transfers, and proper resources for possible discharge from er to home. Educating patients and families/caregivers on disease process, medications administration, and evaluating for optimal outcomes and plan of care indications when have additional concerns and change in plan of care.
