Supports the patient and family at the time of discharge and dying process. Attends required in-service and mandatory departmental meetings. Participates in the collection of data for QI. ESSENTIAL FUNCTIONS:

Practices within the established standards, policies and procedures.

Establishes an initial nursing history, which includes a psychological, social and physical assessment.

Initiates and updates nursing care plan, including the discharge goal for each patient.

Performs therapeutic measures properly prescribed and delegated by medical authority.

Cooperates and shares responsibility of patients care with other staff nurses, head nurses, supervisor and attending physician.

Identifies nursing care and needs of patients.

Carries out doctor's orders accurately and promptly.

Recognizes and interprets symptoms intelligently, plans and implements nursing care and interventions in conformity with established hospital standard and policies and continuously assesses their effectiveness.

Documents all nursing care activities reports by utilizing the problem solving approach.

Assist with the transfer of patients within the health care and outside the facilities. SECONDARY FUNCTIONS:

Follow the standard for infection control; hand washing, protective eyewear, mask, gown, gloves, head cover, and shoe cover, .as appropriate and proper disposal of contaminated items and the use of isolation.

Participate in monthly outdating of medications and supplies, checking of crash cart, intubation tray, and CVP insertion tray. Check, order and stock supplies according to par level.

Adhere to the visitor and nonsmoking policies.

Complete patient charges daily.

Enter orders into the computer.

Participate in chart review. Collects data for QI.

Attend monthly mandatory staff meetings, and mandatory in-service.

Enhance professional growth by attending educational opportunities pertaining to nursing.

Initiate and coordinate a Code Blue.
