Faith Health Services. March 2012-present. Assessment Nurse/Clinical Nurse Coordinator. Initially responsible for completing the MDS assessments for new admissions and re-evaluations for existing clients. In 2015, split my time between completing the assessments, and working in the office 5 hrs a day, hiring new CNAs,
Checking Medicaid eligibility, auditing the ICWP and CCSP charts, and screening clients for the SOURCE program.
Guardian Homecare. February 6, 2012 to April 2014. Home Health Nurse. Responsible for providing treatments in patients homes according to plan of treatments established by the RN Supervisor and physicians. Providing patient teaching regarding medical diagnosis, medications and wound care to patient and to supporting family. Also attending weekly interdisciplinary meetings with other nurses, therapists and social worker to coordinate care for all patients.
Source Care Management. February 7, 2011-present. Assessment Nurse. Responsible for going into members homes conducting face to face MDS assessments to assist them with obtaining community services to prevent nursing home placement. Coordinate personal support services, serve as an advocate for the members, assist with obtaining medical equipment, scheduling doctor appointments, transportation to those appointments, and coordinating family education to assist with providing daily care to members. Also educate some members on disease management and medication.
UniHealth Pruitt. February 2, 2008- December 21, 2010. Assessment Nurse. Responsible for providing case management to members on the Source Program. Also conducted all new assessments and reassessments on current Source members. Performed MDS assessments in the members home to evaluate services needed for members in their homes. Provide support for families of the Source members to effectively care for members in the home setting.
Tender Healthcare. November 17, 2005-February 1, 2010. LPN. Responsible for picking up medically fragile children on the transportation van and accompanying them to day program daily. Assessed each child daily, gathering temp, b/p, pulse, respiration, and at times pulse ox. Administered medications as ordered by their physicians. Upon arrival to day program, assisted in transporting them inside the facility and further assisting with ADLs throughout the day.
PSA. May 2003-November 2005. Private Duty Nurse. Responsible for conducting care for children in their homes. Administered medication, hourly charting on child's performance, assisted with ADLs and IADLs. Accompanied children to physician appointments, conducted patient teaching to family on how to care for children. Provided trach care to some children, vent care, g-tube feedings, O2 therapy, and assisted with Range of Motion as needed.
Kennestone Hospital. November 11, 2001-January 2004. Med-Surg Nurse. Responsible for administering medication, charting on patients, head to toe assessments conducted, assisting with ADLs and IADLs, provided excellent customer service and patient education as needed, drawing blood, and assisting with any other care needed.
