DON at Lebanon Country Manor. 95 bed facility. Focusing on increasing acuity. Preferred provider for wounds and pulmonary. Immediately started cleaning up the population from active drug users (residents and staff). Worked very closely with the Board of Nursing and Warren County Drug Task Force. 10 minor citations for a desk review after annual with State and Feds. Now have a very low turnover rate, did away with agency, more teamwork, lower RTH, continuing to work on lowering falls, have lowered the number of falls with injury, and several unsubstantiated complaint surveys (4 in 10 months).

12/15-5/16 HCR Manor Care

DON at Heartland of Miamisburg. 120 high acuity (MedBridge and long term care) beds specializing in wounds. 1 minor citation during annual survey in 2016. Population: wounds (several amputations), IV drug users, brain injury. Goals: lower RTH rate with education and staff retention/morale.

11/14-12/15 Parkside Nursing and Rehabilitation Center

ADON. Enrolled in the DON in training program. 76 high acuity (skilled, long term care, and ventilator) beds. Population included bariatrics, dementia, IV drug users, sexual offenders. Responsible for scheduling, employee retention, chair of falling star program, organized and lead “Safety Club”, infection control/staff and resident immunizations, staff evaluations, education, Coumadin, glucometer, MAR/narcotic audits, and interviewing. Also work closely with all department heads, medical director, hospice, and the state surveyors. Assisted in creating a Greater Cincinnati chapter of ODONA. Sent to another Embassy building to train the DON and ADON. Perfect VA and annual state surveys in 2015.

12/12-11/14 The Lodge Care and Rehabilitation Center

RN supervisor of entire building. Responsible for 1:1 with 20 of my own residents as well as overseeing the entire building of 118 beds, staffing during my shift. Work very closely with physicians and three hospice groups, responsible for entering and discharging orders, assisting any staff with needs that may transpire, run codes, all port access and PICC flushes, admissions, discharge, increase employee and resident morale, Coumadin therapy control/protocol. Precept RN role transition students several times throughout year.

08/2007 – 11/12 Cincinnati Children’s Hospital Medical Center (CCHMC)- A4N:Transplant/Surgery

Assigned to a twenty-four bed unit, played a key role in organizing employee appreciation events. Assisted in scheduling of 80+ staff members with a team of two other RN’s, manager, and schedule facilitator. Communicated well with other RN staff, physicians, social workers, nutritionist, pharmacist, lab technicians, transplant coordinators, wound and pain teams. Actively participated in development and implementation of individual treatment plans for patients with broad range of medical issues from needing an organ (kidney, liver, small bowel/pancreas) to weight loss and appendectomies. Annual project as Education Council attendee was deciding what PCA and RN annual training classes (Blitz) education would be covered. Often came up with Blitz lesson plan, decided who would be the best teacher, and how to make each lesson as hands on or involved as possible to keep everyone’s attention. Selected as one of only a handful of RNs to attend special classes that allow RN to become a floor educator on special topics when hospital could not afford to send every staff member. Helped refine unit policies and procedures as a nurse practice council member in the areas of safety equipment and JCAHO guidelines. This is included several monthly audits (bed side safety equipment accurate for each patient’s age and weight and in proper place, documentation and labeling to JCAHO standards). Lead of several Code Blues.
