Lead the Regional End of Life Option Taskforce. Ensure that all aspects of program rollout and operate smoothly, including new job roles, staff training, legal, compliance, pharmacy, and data management. Facilitate inter-area Coordinator meetings to provide a venue for collaboration and promoting self-sufficiency.
Designed and implemented a big data, machine learning model to predict a “time to future disease state” based on past cases. First output and quality demonstrated for chronic kidney disease, with plans to expand to more diseases. Estimates are improved with time.
Help to design and build a panel management tool for nephrologists and care managers, including a daily snapshot of patient’s dialysis preparation, GFR and other labs, and risk scores based on literature. The tool is being piloted in clinic.
Partner with physician leadership for analytical and communications strategies to reduce physician’s in-EMR emails via workflows, email etiquette training, and spreading locally-sourced successful practices. Maintain and present a performance dashboard for distribution to Chief Administrators and other local users to compare performance and identify opportunities.
