Worked with a multidisciplinary team to provide assessment, planning, implementation, coordination, evaluation and monitoring of patients for either return to Active Duty or transition to the civilian workforce. Provided advanced practice clinical nursing, administrative and organizational skills in managing the continuity of care for the Wounded Warrior. Conducted a comprehensive assessment of the soldiers health care needs in order to develop a plan of care. Created care plans with the family, primary care provider and other health care providers. Educated both the patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources and other benefits available so that informed decisions could be made. Encouraged appropriate use of healthcare services and strove to improve the quality of care and maintain cost effectiveness. Empowered patients and served as an advocate for patients, healthcare team and family members. Attended multidisciplinary meetings, provided oversight and input to nursing practice matters. Coordinated with multidisciplinary teams of clinicians to develop a time line protocols and clinical guidelines which delineate the expected process of care. Evaluated work performance of subordinates. Interviewed candidates for vacant positions, recommended promotion or reassignment to such positions. Identified training needs of employees. Developed performance objectives; and approved leave. Directed and educated staff about organizational policies and initiatives, The Joint Commission (TJC) standards, and the Medical Command (MEDCOM) policies. Listened to and resolved complaints from patients and staff, additionally effecting reward and disciplinary measures to assigned employees.
