Behavioral Health Autism Forms

The Autism Behavioral Health Treatment Application has been requested with the goal of monitoring the services and treatments administered to autistic patients, who are IEHP members, from providers. This form is utilized as a means of tracking the autistic patient’s progress and growth over time. The Autism BH Treatment Application gives access to three types of forms that are to be filled out and submitted. These forms are known as the Initial Functional Behavioral Assessment Form, ABA Progress Report Form, and 6 Month ABA Request for Additional Services Form.

eAuthorization Request Form

eAuthorization Request Form allows PCPs, Specialists and Vision providers to submit referral authorization requests to IEHP for processing. These referrals are submitted to the MedHOK application for Authorization Review.

Prescription Drug Prior Authorization Request Form

Prescription Drug Prior Authorization Request Form allows providers to submit Drug request to IEHP for processing. These drug request are submitted to MedHOK application for review and processing.

Responsibilities:

Participated in requirement gathering & impact analysis with business users for each Forms.

Medicine ordering system is an application to enable users for setting up the Order workflow, generating allocations for medicine, place/ change/ cancel/return order for the medicines. My Account section of the MOS allows users to view profile information and to look up current or past orders. It automatically sends an order confirmation to the configured email address for the user when the order is created. This is equipped with a suite of standard reports that are available to view on-screen and can also be printed. Various custom reports can provide information on orders, products and billing details.
