From - 2000 to present Position – Staff LPN

As a staff nurse for LHHH my current position has been to make skilled nursing visits in rural Vermont to a population ranging from pediatric to hospice. During these visits we follow a plan of care which is adjusted as needed per MD orders. We are continually assessing and advocating for the clients current and future needs, making the appropriate referrals to other professionals or agencies. Keeping in mind that it is also important to support not only our clients but their families as well. Referrals to LHH may be for many reasons such as client safety, venipuncture, medication management, post op assessments, wound care or hospice. I have also worn other hats during my years with LHHH. I have worked in QA/QI, reviewing charts for compliance and clarity. Represented LHHH at discharge planning at our local hospital to coordinate a smooth transition from hospital to home. Worked as an intake nurse, accepting referrals, checking insurance eligibility and benefits and have worked with our LNAs to prevent work related injuries.

I feel that my strongest attributes are professionalism, dependability, my ability to work independently but also within a team and the ability to communicate effectively with clients and other healthcare professionals.

From Jan 2016 to Mar 2016 Position- Worked on a yearly, temporary contracted Medicare CMS project as a medical record review abstractor.

For this position, I worked at corporate headquarters in Phoenix. We had to learn Banner Health’s three different charting systems within two weeks to abstract information from charts required for the project. Speed and accuracy were of the utmost importance. As a team of six, we finished the project one week early.

From 1995-1997 Position - I worked as night supervisor, prior to that I worked as a staff nurse

Lakeview Neuro Rehab is in NH situated on a mountain. Its 80-90 clients ranged in age from 5 to 65. All clients suffered from various traumatic brain injuries with the many different behavioral and physical challenges that it can encompass. As night shift supervisor I was responsible for staffing, schedules, employee reviews and teaching staff and client safety. I was also responsible for keeping up with organizational safety requirements such as monthly fire drills. It was also a part of my duties to be available to assist as an LPN to administer medications and assess/support the clients/staff as necessary.

Prior to and in between ---

I have worked as an LPN in long term care, sub-acute rehabilitation and on a med- surg unit in a hospital setting. Nursing was a second career for me. I have been in the US Air Force as an A1C working as a C 130 turbo prop and jet engine mechanic and have worked with the US Government in a GS position as a photographer for historical documentation at Rhein Main AFB in Frankfurt Germany.
