Career History & Work Experience:

Fortis Malar Hospital, Chennai, India: November 2013 to date

Presently working as Consultant in the Department of Cardiac Anaesthesia & Cardiac Critical Care at Fortis Malar Hospital, Chennai, a 180 bedded corporate hospital doing both routine complex congenital & adult cardiac surgeries (600 surgeries/ year) as well as India’s first Comprehensive Heart Failure Centre performing ECMO (20-25 cases/ year), Heart Transplant (15-25 cases/ year) as well as Left Ventricular Assist Devices implantation (15 patients underwent Heart Mate 2 or HeartWare implants in last one year).

As part of Heart Failure Clinic, Job description includes but not restricted to

Pre Operative work-up and optimization of heart failure patients, including detailed Transthoracic Echo (TTE) as well as Right Heart Cath Study of all patients. Please note that we do not have a heart failure cardiologist as part of our team presently and all this work-up is done by the cardiac anaesthesia team. After patients are assessed, they are optimized with inotropes, diuretics as well as IABP if necessary. We have put very sick patients on veno-arterial ECMO and successfully transplanted them after a few days.

Intra-operatively, patients are managed with advanced haemodynamic monitoring including Continuous Cardiac Output PA Catheter with SvO2, Flotrac, and Trans Esophageal Echo (TEE).

Post-operatively, all patients are managed by the same Cardiac Anaesthesia & Critical Care team and we have acquired considerable experience of managing immunosuppression for heart transplants and have streamlined our VAD management protocols. We also have a large experience of using ECMO for heart failure, acute cardiogenic shock as well as sepsis/ARDS.

Fortis Malar Hospital currently has the busiest heart-lung transplant program in India and we are presently the only hospital in India to have successfully done a VAD implant. We now have many heart transplant and VAD patients on regular follow up with us.

In addition, all the routine adult and complex pediatric cardiac surgeries are carried out including coronary artery bypass grafting (CABG), valve repair and replacement as well as lung and vascular surgeries as well as closure of atrial and ventricular septal defects (ASD/VSD), repair of tetralogy of Fallot (TOF), repair of total anomalous pulmonary venous connection (TAPVC), arterial switch operation, the Senning operation, the Glenn procedure, the Fontan procedure (including the extra-cardiac Fontan operation),, repair of common AV canal defects, the Rastelli operation, PA banding, BT shunt and conduit revision.

Training and work experience in TransEsophageal Echocardiography:

Department of Anaesthesia, fortis malar hospital, Chennai, has the modality of TEE in cardiac operation theatres.

Residents are trained in basic monitoring using TEE in adult and pediatric cardiac patients. This includes getting acquainted with all standard TEE imaging windows, use of color Doppler, flow mapping, PW & CW Doppler, appraisal of valvular integrity, grading stenotic and regurgitant valves, assessment of ventricular function, regional wall movements, ischemia detection, functional assessment of RV, quantifying cardiac output and evaluation of diastolic function (including tissue doppler) and making therapeutic decisions intra-operatively based on TEE findings.

As Clinical Assistant in Cardiac Anaesthesia and Critical Care it was routinely using TEE in both adult and pediatric cardiac patients both intra-operatively as well as in the cardiac ICU.

As Junior Consultant at Fortis Malar Hospital, have gained proficiency in TTE as all heart failure patients are assessed by the cardiac critical care team. We are also using TEE routinely intra-operatively (Philips iE 33 machine) in all our cases, and assessment of RV function in particular is done in detail in VAD and heart transplant patients.
