Duties and Accomplishments

As a PRIMARY YOUTH COACH, I work with 12 to 18 years old youth, to encourage school success either by successful graduation or completion of a GED program and to prevent potential involvement with the Criminal Justice System. I work with local schools; youth services counselors, local law enforcement and other individuals to identify youth needing enhancement services. I evaluate youth’s progress by obtaining goals and objectives, and reviewing the client’s case at least one time per month at case plan meetings and more often as warranted by the youth’s behaviors both during and outside of the program.

As a CASE MANAGER, I served delinquent youth ages 12 to 17 years old that were at a high risk of becoming further involved in the Criminal Justice System by offering re-integrative theories and practices. I served by providing day treatment/group counseling, individual counseling, recreational therapy, role playing, supervision (curfew) and family intervention. I taught family sessions that taught parents effective parenting skills and strategies to help them understand their child's behavior. In addition, I addressed issues involving the adolescent's continuing educational and vocational training. As a counselor, I trained youth in job readiness skills, including job applications and interviews. Other program activities and training include addressing issues of sexual activity, sexually transmitted diseases, personal hygiene, responsible parenting, trust and, self-esteem. The treatment modalities and intervention goals that I taught consisted of decreasing criminal activity, encouraging school attendance or obtaining a G.E.D. facilitating goal development toward employment, inspiring students to be more positive and effective individuals. Ensuring youth will be cared for in their homes or in the homes of relatives. Preventing and reducing out-of-wedlock pregnancies. I planned, developed, and maintained a network of relationships with community resources such as youth court, mental health, school systems and other advocates for the juveniles I served. Developed treatment plans, documented youths assessments, and progress notes.

As a FAMILY PROTECTION SPECIALIST, I rehabilitated and preserved families and provided protection and care for children in need of supplementary or substitute care, and provided full-time intake services. I evaluated the social needs of the children, developed a plan directed towards the solution of child and family problems, and provided protective and placement services for homeless, dependent, or neglected children. I participated in case conferences and/or staff meetings. Established and oversee administrative procedures to meet objectives set by boards of directors or senior management. I provided training to clients and other social workers. I completed special assignments, investigations, and training requirements. I provided case management services. I performed related or similar duties as required or assigned. I coordinated counseling efforts with mental health professionals and other health care professionals, worked with mental health clients make referrals to other community/agency resources for clients. Made direct contact with families, as well as being on-call 24/7 to provide crisis intervention as needed. Monitoring individual and family progress in attainment of predetermined goals, as well as providing regular review and updating of service plans as needed. Working to develop and maintain effective working relationships with families and community service resources.
