JOB DESCRIPTION:

Direct delivery of the neonatal patient intensive care by promoting good standard

of nursing practice through patient centered care by: assessment, planning,

implementation and evaluation of nursing care rendered.

Assessment: Assess patients problem and plan the nursing intensive

care as needed. During endorsement time, reports patients condition at

bedside. Assess patient from head to toe during taking over. Assess IV

cannulation site, correct IV fluids, IV drip rate (IV therapy) date of insertion

and label. If the patient is on ventilator endorse about the settings, tube

size date of insertion, chest X-ray taken etc. Assess wound dressing if

any. Check all patients file if documentation is updated and recorded, flow

chart, medication sheet and other necessary charts.

Planning: Prepare patients file for Doctors round, patient admission,

paper consultation form, history and physical examination record,

progress note, doctor order sheet, laboratory investigation result, radiology

result and ECG. Keep the previous day flow chart, laboratory results, ABG

and X-ray together with the clipboard of each patient. Carry out all plans of

intensive care and doctors order without delay.

Implementation: Check patients VS, record and nursing action taken.

Attend to new admission of patient by taking VS, body weight, carry out

Doctors order, inform dietician and record patients addressograph in the

admission record book, census form and midnight census. Assist the

Doctor during rounds to the assigned patients, carry out all order and sign

in the order sheet. Sees to it that all carried out correctly. Carry out all

investigations ordered during rounds and observe the VS. Check blood

glucose level of patients if necessary, extract blood per Doctors order,

send to laboratory and ensure that all blood and laboratory results are

seen and signed by the attending Doctor. Administer medications correctly

following the Medication Administration System Policy. Ensure that the

Doctor entered the prescribed medication in the computer and collect the

medication from the ones ready. STAT orders must be collected

immediately. Attend to mouth and eye care, chest physiotherapy and

suction ever 2-3 hours. Attend personal hygiene of patient and ensure that

patients are clean and tidy. Attend and assist Doctor in any procedure

such as Endotracheal Tubing, Umbilical Cord Catheterization, Chest Tube

Insertion, performing Lumbar Puncture, etc. Be sure all the equipments

and materials needed are ready and available such as: Ventilator machine

and tubings, Sipap, dressing set, forceps, syringe pump, cardiac monitors,

suction machine, etc. Serve bottle feeding for infants who require feeding

and assist feeding if required.

Evaluation: Evaluate nursing intensive care rendered, reevaluate and

implement plan of intensive care if necessary. Discuss with head Nurse

and Charge Nurse in the evaluation and implementation of intensive care

whenever necessary. Evaluate patients condition and notify Doctor if

necessary. Inform Nursing supervisor if needed for transport going to other

hospital. Refer to social worker for any social problems to deal with.

Abide to Infection Control Policy.

Provide patients right and confidentiality.

Provide patient/family education according to patients needs. Provide transfer out

or discharge planning.

Ensure that all documentation completed is both accurate and legible.

Assess patients condition and VS. Ensure patients file completeness from

if any results needed urgent medical attention.

Monitor blood sugar level while patient on short term treatment;
