Summary of Experience

REGISTERED NURSE/LICENSED PRACTICAL NURSE

Assess patients to make out suitable nursing interventions; Oversee medications and treatments as approved by a physician or in keeping with established policies and protocols

Work together with members of an interdisciplinary healthiness team to plan, put into practice, organize and appraise patient care in discussion with patients and their families

Oversee LPN licensed practical nurses and additional nursing staff

Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts

Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action

Answer patients' calls and determine how to assist them

Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration

Provide basic patient care or treatments, such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or performing catheterizations

HOSPICE CASE MANAGER

Support and counsel clients who are facing terminal diseases and conditions

Coordinate a collaborative hospice care team that may include healthcare providers, at-home care services, mental health specialists and spiritual advisors

Evaluate a patient's physical condition and develop a care plan that addresses any medical needs and social services

Coordinate the efforts of medical and nursing team members to provide appropriate care

Address the psychological needs of patients and their families through counseling and education efforts.

Provide dialysis nursing care to filter toxins and waste products from the blood for patients with kidney problems as ordered by nephrologists or physicians

Arrange dialysis equipment according to patients’ requirements; Maintain and manage dialysis machines, systems and equipment

Oversee dialysis procedure from the beginning to fruition; Monitor and assess vital signs of dialysis patients; Document patient progress and manage reports

Prepare dialysate and assemble extracorporeal circuits; Manage cleaning of catheters

Identify problems or irregularities in the dialysis procedure

Provide post care and pre care to dialysis patients; Instruct and counsel patients & their families on dialysis nursing care issues

Administer medications as prescribed by nephrologists or physicians

Ensure comfort to patients while executing dialysis nurse functions
