Assists the Case Managers and the Director of Nursing to conduct infection control surveillance activities and client or employee grievances.
Participates in evaluating the effectiveness of the services provided in consideration of the desired outcomes. Care Coordinator to contact clients to initiate home health aides schedule as directed in plan care. Continue ongoing communication with clients and or family members to ensure the quality of care is being met.

Facilitates plans for the client’s discharge from service and assists with the final coordination of care activities to ensure discharge occurs as ordered.
Participates in on-call activities and responds to unexpected scheduling problems, afterhours, office closing and during holiday schedules.
Performs other duties as assigned by the Director of Nursing facilitate delivery of client health care services. Report all clinical issues to the Clinical Director.
