Company founded by 9 Central and Northern Illinois hospitals to provide Care Coordination for Medicaid patients within an ACE (Accountable Care Entity). Company goals are to become an Insurance plan by July 2016.
Medicaid experience per the State of IL requirements – IPH forming partnership with local MCO.
Coordinate meetings to ensure smooth transition of membership to this newly formed partnership.
Familiar with BIG DATA tools and how they help with Population Health Management.
Work with Care Coordination team at IPH to help them meet their compliance reporting needs.
Responsible for maintaining and managing membership/enrollment data file feeds.
