Serve as the primary Community Outreach Specialist, conducting outreach to 21 long-term care nursing home facilities explaining the transition process to assist with home and community base services. Target seniors 60 and older and 18-59 with disabilities about the resources and procedures for securing an independent life in the community; proven advocate (liaison) in collaboration with designed agencies, staff and stakeholders to assess the needs of individuals.

Responsible for participating in interagency programs to provide informational sessions for individuals living in long-term care facilities, their families, and guardians, on a monthly and quarterly basis.

Selected Accomplishments:

Complete detailed assignments working as lead Community Outreach Specialist to represent D.C. Office on Aging (DCOA)/Aging and Disability Resource Center (ADRC) and the Department of Health Care Finance in discussions with other DC government agencies, the community at large, and other interest groups;

Ensure that people who are living in an institutional setting are connected to the correct process, agency, while providing educational materials; brief staff on reports and strategically plan and implement solutions to creatively ensure that the process is place before transitioning;

Manage a wide variety of customer service and administrative tasks to resolve customer issues quickly and efficiently while effectively responds to e-mails, calls orally and in writing to schedule, and address inquiries.
