06/2016-Present City Of Oakland-MSSP Case Manager II

Conducts initial psycho-social assessments and provides intensive case management to seniors in the community over the age of 65 years old to prevent out of home placement to skilled nursing facilities or intermediate care facilities. Provides assistance with financial resource management, crisis management, service delivery planning and follow up; and referrals to appropriate short or long term counseling when indicated.
